To help demonstrate the benefits of my services and the different ways in which they can be tailored to meet your needs, I have given some examples below

Situation:

​

• Lives in an apartment within a Retirement Living Development

• Lives with Dementia

• Son and Daughter-in-Law live 30 minutes drive away

 

Challenges:

​

• Living with Dementia is affecting her ability to buy food, cook safely and manage medication

 

Existing support network:

​

• Son and Daughter-in-Law manage appointments, source service providers and problem-solve as well as making social calls and taking for trips out

 

Services provided by And Wiser

 

Support Package:

 

• Twice-Weekly Support Visits

• Problem-solving assistance for family by suggesting and sourcing potential solutions for problems e.g. cameras, trackers and lockable switches for cooker and microwave

• Shopping for groceries and household supplies, ensuring familiar brands are supplied

• Managing supplies so that there is just enough in the home, but not too much (as this triggers items to be thrown out as they are incorrectly thought to be old and out of date)

• Liaising with Care Provider and Carers to manage meals, laundry and medication and deal with any problems or changes required

• Regular communication with family after every visit and as required in between visits

Situation:

​

• Lives in a suburban semi-detached house with no amenities within walking distance

• No longer drives due to health reasons

• Niece lives more than 3 hours drive away

 

Challenges:

​

• Unable to get to shops, appointments or enjoy trips out without support

• Finds it difficult to use the phone and deal with organisations, especially when problems crop up

• Finds it difficult to stay organised with an increasing number of appointments and health professionals to deal with

 

Existing support network and circumstances:

​

• Neighbours do weekly grocery shopping and deal with wheelie bins

• Weekly cleaner, who also changes bed and takes bedding away to wash, dry & iron

Support Package:

 

• Weekly Support Visits

• Problem solving service to respond to ad hoc needs in between visits

• Sourcing service providers when required

• Organising appointments and keeping on top of associated paperwork

• Errands & Shopping as required

• Monthly email update to Niece and ad hoc liaison as required

 

Additional Ad Hoc Services Provided:

• Transport provided and accompanied to appointments

• Trips out to enjoy shopping, visiting cafes and walks in nature

• Crisis support during sudden illness, providing extra visits and ensuring appropriate health services contacted

• Project to source and organise a care package and then ongoing liaison with care provider once in place

Situation:

​

• Lives in an apartment within a Retirement Living Development with social activities provided on site and town centre amenities within walking distance

• No longer drives due to health reasons

• Adult children all live more than 3 hours drive away

 

Challenges:

​

• Now lacks confidence to go out without support since mobility and balance have deteriorated

• Not confident using taxis for appointments as feels the need for support all the way in and out of buildings

• Sometimes misunderstands what action is needed and gets flustered when new correspondence is received so would prefer family to deal with anything official, but unable to use technology to send copies quickly.

• No longer able to pop out to run errands and pick up misc shopping

 

Existing support network:

​

• Weekly cleaner, who also changes bedding, does laundry and grocery shopping

• Adult children help with decision making by phone, and help with other tasks in person when visiting a few times a year

• Monthly Support Visit

• Problem solving service to respond to ad hoc needs in between visits (in particular to help interpret correspondence and send copies to family as required)
