SUPPORT AT HOME PACKAGES

 

Support Packages are tailored to meet your needs, and include flexible ad hoc support.  This means I will call in, as and when required, to run errands and deal with any problems that crop up in between scheduled visits

 

Packages can include features such as:

ADDITIONAL SERVICES & PROJECT WORK

A subscription to a Support Package also gives you access to additional pay-as-you-go services and project work, for example:

 

  • Accompanied appointments with transport provided

  • Trips out to enjoy shopping, visiting cafes, walks outside in nature

  • Crisis support with extra visits and liaison with health services

  • Supervision of tradespersons

  • Technology support

  • Support with hobbies and social activities

  • Decluttering and home organisation

  • Sourcing, onboarding, ongoing liaison with a care provider

Home Support Visits

A regular visit in your own home

During the visit I can do some pre-agreed tasks or you can decide on the day what you want me to do.  For example I can help you with any problems that have cropped up or help you deal with paperwork.  It may also be possible to facilitate video calls if you'd like to speak to relatives while I'm there to support with the technology.

Phone Support Calls

A regular scheduled phone call

Instead of (or in addition to) Home Support Visits, I can arrange to call you on a regular basis, to have a chat and find out if there's anything you'd like me to do for you.

Phone Helpline

As-required access to phone me for support (during specified times/days)

I will respond to calls directly when possible or ring you back within a specified period.  This gives you the option to ring me instead of your relatives or neighbours if problems crop up, if you have a question or just need a quick chat about something.

Support with Problem-Solving & Sourcing Service Providers

If you find it frustrating dealing with some day to day problems e.g. Curtain rail falls down, TV stops working, button falls off cardigan etc.  Rather than having to phone a relative or neighbour, or wait for them to visit, I can help you sort it out and, if possible, find a way to stop it happening again or put the right support in place for the future.  This would include sourcing any service providers required.

Organising Appointments

These can become difficult to keep on top of, especially healthcare appointments, when more than one department or specialist is involved.  I can help you keep on top of your paperwork, update calendars, communicate with appointment providers and family, and support with arrangements for attending appointments (e.g. organising patient transport, taxis, accompanied appointments) and organise follow-up appointments.

Shopping Support

From weekly grocery shopping to gifts and household items, I can make arrangements to cover regular and ad hoc purchases.  I can do this for you, or organise something to suit your lifestyle e.g. for regular purchases such as groceries, this might mean arranging regular shopping trips with a Companionship Service, or internet shopping to be delivered during a scheduled visit from a cleaner who can help unload and put away.

Dementia Support

If you're living with dementia, you may have some very specific needs, to help you stay living safely and happy at home for as long as possible.  The support package can be tailored to meet your needs and I will remain flexible and responsive to changes over time so that we can adjust services as required.  With regular contact over time we will get to know eachother and build a relationship which will help us work through any challenges that arise.

Liaising with Care Providers

Even if you have regular visits from Carers, there will still be issues to deal with on a regular basis, e.g. Carers may ask for household equipment to be replaced such as needing new oven gloves or a mop, and any disposable supplies will need to be replenished regularly such as wipes and incontinence pads.  There might also be problems to deal with if Carers appear not to be carrying out tasks correctly, and if your needs change, it's important to ensure that they're communicated effectively to everyone involved.

Holiday/Illness Cover

If you get any regular support from relatives or neighbours then if they are ill or going away on holiday, I can increase the level of cover I provide on a temporary basis e.g. with extra visits or calls.

​Family Update/Support Calls

A regular phone or video call (between me and your family members)

If you would like certain family members to be kept up to date, these calls are an opportunity for me to update them on how you are doing and discuss any problems.  I can also update them with progress on any additional projects I'm working on for you.

Family Messaging Group

A private group set up on an App like WhatsApp for ad hoc communication

If you would like certain family members to be kept up to date, I can set up a private messaging group with family members on an App like WhatsApp for quick questions and updates by text or voicenotes in between scheduled calls.
